Instructions on how to fill out the application form

<UPPER PART>

1.      Check □ with ✔ according to the test levels on ①.

Check only one between TOPIK I or TOPIK II,

2.      ②, the application number, would be also given during the application. Do not write anything on the blanks.

3.      Write the province/area where(MANILA) you will take the test at ③.

4.      Photos with the size of 3 × 4㎝ and printed on photo paper must be attached on both designated

places. The photos must be printed in color and must be recognizable. Photos in black and white will not be accepted.

6. Fill ④ with name in Korean character from surname to given name. If you don’t have Korean name, you don’t have to write it down.

7. Write a name in capital in ⑤. The name must be written exactly as it appears on passports or official identification cards. Write down from your surname to given name.

8. For ⑥, check □ with ✔ according to the gender of the applicant.

9. Write the nationality in English on ⑦. Check ( ) with ✔ if you are a Korean living overseas.

10. For ⑧, check □ with ✔ according to the occupation of the applicant.

11. Write the date of birth with the format (yyyy/mm/dd) in ⑨ and write the age in ( ).

12. Write the address and contact number on ⑩.

13. For ⑪, check □ with ✔ according to the motive of the application

14. For ⑫, check □ with ✔ according to the purpose in taking the TOPIK.

15. A new section has been added on the application form.

   Read it carefully and affix your signature if you permit NIIED to use your personal information for

TOPIK

<IDENTIFICATION (APPLICATION) SLIP>

1.      Identification (application) slip must be filled out with the same information from the upper part of application form.

2.      ⑭ The application numbers on the application form would be given during the application. Do not write anything on the blanks.

3.      For ⑮～ ⑳, write the same as in the application form above.
